<p class="wp-block-paragraph">Digital devices have become integral to modern life; almost to the point that for some people they have become an extension of self. It is now close to impossible to imagine not having such devices immediately to hand wherever we are, day or night. They can seem and may now even be, in some respects, indispensable. But is their portable convenience also problematic for some of us?</p>



<p class="wp-block-paragraph">A “consuming relationship” is one where, in the process of consuming, you also become consumed, thereby losing a degree of autonomy. Much more time and energy than is actually needed is given over to interaction with the device. This bears some similarity to the substance addicted person whose life becomes dictated by the need to secure and consume their drug(s) of choice.</p>



<p class="wp-block-paragraph">Reflexes become increasingly conditioned and automatic, linking stimulus directly to some sort of reward, ranging from the simple discovery of what’s prompted an alert, to the tantalizing promise of access to all that can be transacted on line. You may have already noticed these “withdrawal symptoms” if you have ever mislaid your phone (even briefly), lost access to charging or when social etiquette requires it to be switched off. <p class="wp-block-paragraph">You can also assess your relationship with your device by answering questions in these areas as honestly as possible:</p>



<h3 class="wp-block-heading">TIME</h3>



<p class="wp-block-paragraph">Depending on how long we sleep, we spend roughly 16 hours of the 24-hour clock awake. How much of that time is spent on the device? When you’re not using it how much time is spent thinking about using it?&nbsp; What is done with the time spent when you do? Is it quality time (i.e., healthy and productive) or mindless and aimless as you drift from one thing to another? Do you lose track of time and even forget when, where and why you began? If you had a clear purpose, is it one that simply enabled an addiction such as to gambling or pornography?</p>



<h3 class="wp-block-heading">CONTROL</h3>



<p class="wp-block-paragraph">At first, as with substances or behaviours which become addictive, we make a choice to use something that gives us a sense of control over what we feel. Over time it can feel less and less like a free choice as it turns into a compulsion; that is, it comes to feel next to impossible not to (choose to) do it.</p>



<p class="wp-block-paragraph">When it comes to something like a smartphone, it is worth asking yourself the following: Do you feel in control of the device or do you feel controlled by it to some extent?&nbsp; (Bear in mind that there is a tendency to deny loss of control as it can feel shameful and provokes anxiety.) Do you feel compelled to check your phone whether it alerts you or not? Does this checking happen a great many times during the day and often when there is no real reason for you to do so? Do you check more often than you feel comfortable with doing so? Do you feel compelled to check it immediately it alerts you no matter what you are doing? Would you like to feel less attached to the device (even free of it) and do you spend time thinking about what life would be like if you were? Do you find it difficult to see how you could be less attached?</p>



<h3 class="wp-block-heading">INTRUSION, INTERFERENCE, DISTRACTION</h3>



<p class="wp-block-paragraph">Due to its easy portability, the phone is — a bit like a pack of cigarettes for a smoker — ever present, often including being placed next to the bed or on the meal table. As it may awaken and grab your attention at any time, it can come to feel intrusive. Rather than remaining a subservient aid to your life, its use may begin to interfere with activities and real-world relationships.</p>



<p class="wp-block-paragraph">During time spent with someone else do you still feel compelled to look at the phone as soon as it alerts you and even when it doesn’t? When participating in a much loved activity, are you easily distracted from it by the phone? Is it impossible to wait? If you do manage to postpone your check, do you lose focus and find yourself speculating on what the phone has alerted you to?</p>



<h3 class="wp-block-heading">ADVERSE CONSEQUENCES</h3>



<p class="wp-block-paragraph">The consequences of a consuming relationship may be felt by you and by others close to you. You may feel depressed, apathetic, demotivated and less productive. You may feel forever distracted. You may lose restorative sleep. You may find any online addiction intensifies through ease and frequency of access via the device. It is so quick to offer up what you’re looking for, which is powerfully reinforcing.</p>



<p class="wp-block-paragraph">Personal relationships could suffer as others around you can feel excluded, even alienated by your preoccupation, especially if it seems to interfere with the quality of the time spent with you. They may feel as if less important to you than whatever demands your attention via your phone; or that you are bored by them.</p>



<p class="wp-block-paragraph">Perhaps the most serious of consequences is when phone use directly affects yours and other people’s safety. In the UK, for instance, between 2014-2019 there were 133 fatalities and 446 serious accidents due to distraction by mobile phones while driving.</p>



<h3 class="wp-block-heading">IN ADDICTION TREATMENT</h3>



<p class="wp-block-paragraph">One area in which a consuming relationship to a smartphone or other device can be a problem is during treatment for addiction, which is, let us remember, a life-threatening condition. As such, its treatment requires your undivided attention. You will have lost much of yourself to the all-consuming nature of addiction. It will be difficult to re-discover that part of you if time, focus and energy is spent “elsewhere”.</p>



<p class="wp-block-paragraph">To have a meaningful chance of establishing the basis for a lasting recovery you need to be fully present, engaged in and committed to the treatment process; not mentally and emotionally detached from it. An excessive attachment to a smartphone can too easily interfere with the consistency of that engagement. While connected on line you will be disconnected to some extent from the real present world where you are. While present you are also absent. This will make it much more difficult for the treatment team to help you.</p>

<p class="wp-block-paragraph">Sex addiction and pornography addiction — often referred to as Hypersexuality Disorder — have yet to be formally recognised as specific conditions by the American Psychiatric Association in the Diagnostic and Statistical Manual of Mental Disorders (DSM-5). It may be considered compulsive sexual behavior (CSB) when sexual habits become difficult to control, and are disruptive to you or others. The World Health Organization has recommended that compulsive sexual behavior be included in the upcoming eleventh edition of International Classiﬁcation Diseases as an impulse control disorder.&nbsp; Nonetheless patterns of compulsive sexual behavior causing serious problems and distress to individuals and families are very real. It will take time for evidence from research to catch up with the often harmful sexual behaviors reported by patients and observed by clinicians. These patterns and the symptoms they produce appear to align, in many respects, with the diagnostic profile of addiction.</p>



<h2 class="wp-block-heading" id="h-emotional-urgency-and-compulsion"><strong>Emotional urgency and compulsion</strong></h2>



<p class="wp-block-paragraph">In addiction, the ability of whatever substance (e.g., alcohol, cocaine or nicotine) or behaviour (e.g., gambling, shopping, exercise) to change the way a person feels, quickly and effectively, is pivotal. Their attraction lies in their ability to stimulate the reward centres of the brain. Sexual behaviour is one such activity.&nbsp; For some people, the pleasurable effect is so powerful that they do not merely want to repeat the experience sometime in the future, they feel compelled to do so, and soon. There is an emotional urgency akin to craving, especially as they find positive effects on mood wear off too quickly. With the frequency of repetition increasing, the compulsive pursuit of short-term sexual gratification begins to preoccupy the mind, eventually taking over much of everyday life.</p>

<p class="wp-block-paragraph">We are advised that exercise is good for us and of course it is.&nbsp;&nbsp;It contributes to the improvement of health and wellbeing generally but it has also been shown to have a positive effect on mood and self esteem.&nbsp;&nbsp;In light of these benefits, moderate exercise is now frequently prescribed to help ease some forms of depression.&nbsp;&nbsp;However, these same rewarding aspects are what lead some people to develop exercise-related problems. What researchers remain unsure of is when and whether such a problematic relationship qualifies as an addiction.</p>



<h2 class="wp-block-heading"><strong>Similarities with substance and behavioural addictions</strong></h2>



<p class="wp-block-paragraph">As yet there are no officially endorsed diagnostic criteria for exercise addiction. While scientific debate continues, clinicians tend to rely on diagnostic indicators similar to those applied to substance use disorder and behavioural <a href="https://cliniclesalpes.com/blog/can-gambling-addiction-be-cured/">addictions such as those involving gambling</a>, pornography, internet gaming and so on. These indicators include:</p>



<ul class="wp-block-list">
<li><em>Tolerance</em>&nbsp;– needing to engage in more and more exercise to achieve the same desired effects</li>



<li><em>Withdrawal –</em>&nbsp;experiencing unpleasant physical, mental and emotional symptoms when not exercising</li>



<li><em>Loss of Control</em>&nbsp;– the need to exercise is taking over, overriding will and directing choice</li>



<li><em>Intention effects</em>&nbsp;– spending longer exercising or exercising with greater intensity than intended</li>



<li><em>Time spent</em>&nbsp;– an increasing number of hours are given over to exercise, to thinking about and planning it</li>



<li><em>Narrowing of focus</em>&nbsp;– other aspects of life are relegated and diminished in importance</li>



<li><em>Continuance</em>&nbsp;– A person continues to exercise excessively despite mounting evidence of problems, imbalances in normal functioning and harms such as injury and illness</li>
</ul>



<h2 class="wp-block-heading"><strong>When it is time to seek help</strong></h2>



<p class="wp-block-paragraph">High frequency and intensity of exercise do not in themselves indicate an addiction. Concern arises when the motivation for exercising shifts from simple enjoyment or becoming fitter to the relief of distress or dysphoria and to raising self-esteem. The relationship becomes more addiction like if it develops into the sole means of coping and emotional self-regulation. The same concern applies if the organisation of daily life shifts to fit around the demands of exercise rather than exercise fitting comfortably into the day. These developments, taken in conjunction with evidence of injury and illness caused by a depleted immune system, signal that time has come to seek help.</p>



<h2 class="wp-block-heading"><strong>Recovery: from unhealthy to healthy exercise</strong></h2>



<p class="wp-block-paragraph">It is essential to consider that excessive exercise may be part of another problem such as an <a href="https://cliniclesalpes.com/conditions-treated/mental-health/eating-disorders/">eating disorder</a>, as these commonly co-occur.&nbsp;&nbsp;Exercise may be being used compulsively as a means of weight control.&nbsp;&nbsp;In these circumstances, given the potential risks to mental and physical health, the option for residential&nbsp;<a href="https://cliniclesalpes.com/treatment-and-therapies/">treatment</a>&nbsp;where medical facilities are available and well integrated with psychological&nbsp;<a href="https://cliniclesalpes-switzerland.com/treatment-and-therapies/" target="_blank" rel="noreferrer noopener">therapies</a>, should be considered.&nbsp;&nbsp;</p>



<p class="wp-block-paragraph">Abstaining from exercise is not applicable as some exercise is essential to good health. Cognitive Behaviour Therapy (CBT) may be the best model to help the person recognise and self-manage the dividing line between excessive and moderate, healthy exercise. This will require insight into what drives the&nbsp;<a href="https://cliniclesalpes.com/conditions-treated/addiction/">addiction</a>-like behaviour and social support for change.</p>

<p class="wp-block-paragraph">Gambling was the first behavioural&nbsp;<a href="https://cliniclesalpes.com/conditions-treated/addiction/">addiction</a>&nbsp;– that is, one not related to use of substances —to be officially classified as such.&nbsp; It was included in the diagnostic manual DSM 5 under Addictive Disorders.&nbsp; This set an important precedent that led to Internet Gaming Disorder being included under the “Emerging Measures and Models” section; effectively the waiting room for conditions requiring further study before endorsement by the American Psychiatric Association (APA).&nbsp; Meanwhile the World Health Organisation (WHO), in assessing the impact of computers, smart phones and the internet, had singled out Gaming Disorder as deserving of diagnostic recognition and added it to its classifications within ICD 11 under mental health conditions.</p>



<h2 class="wp-block-heading"><strong>The loss of control</strong></h2>



<p class="wp-block-paragraph">As with other addictions, Gaming Disorder is characterised by the diminished ability to control gaming activity as demonstrated by ever greater priority and importance being given to gaming at the expense of other everyday activities and interests. Furthermore, the tendency is for it not only to continue compulsively but also to escalate despite a variety of negative consequences.&nbsp; For a diagnosis to be justified, the severity of the pattern must include a significant impairment in personal,&nbsp;<a href="https://cliniclesalpes-switzerland.com/families/" target="_blank" rel="noreferrer noopener">family</a>, social, educational and occupational functioning, which will, under normal circumstances, have been in evidence for a minimum of 12 months. Many clinicians use this time frame as a guideline rather than as a hard and fast rule.</p>



<h2 class="wp-block-heading"><strong>Five out of nine signs indicates disorder</strong></h2>



<p class="wp-block-paragraph">It is important to note that gaming behaviour must be causing significant impairment and/or distress to warrant a diagnosis. If five or more of the following symptoms are experienced within a year, Gaming Disorder may be identified:</p>



<ul class="wp-block-list">
<li>Preoccupation – a great deal of time is given to thinking about gaming even when not playing</li>



<li>Withdrawal symptoms – emotional and even physical discomfort and agitation is experienced when not gaming</li>



<li>Tolerance – needing to increase the amount of gaming to achieve the same level of stimulation or reward</li>



<li>Attempts to reduce gaming fail –&nbsp; will power is undermined and motivation overridden</li>



<li>Loss or neglect of other interests, even of self-care – evidence that priorities have changed</li>



<li>Continues despite awareness of the negative impact – ignores or plays down the effects identified by self and others</li>



<li>Deceit – lying to cover up the amount of time spent gaming</li>



<li>Used for relief or escape – a way to deal with uncomfortable realities and feelings</li>



<li>Risked losing a significant opportunity or a relationship</li>
</ul>



<h2 class="wp-block-heading"><strong>More research needed</strong></h2>



<p class="wp-block-paragraph">Debate about the diagnostic validity of Gaming Disorder will continue until research provides a clearer indication to support the classification. As with other addictions, one avenue to be explored is how much the behaviour may be a symptom of underlying psycho-social problems. Some clinicians and academics have warned of the danger of pathologising and therefore potentially stigmatising what may be for some people “normal” behaviour, which may even have some benefits to the user.&nbsp; While it is essential to take note of this caveat, significant numbers of distressed people are coming forward seeking help.</p>

Although the recognition of the possible emergence of a new&nbsp;<a href="https://cliniclesalpes.com/conditions-treated/addiction/">addiction</a>&nbsp;dawned almost a quarter of a century ago, hardly any of the apparent disorders related to use of the internet and its associated technology have yet received formal classification.</p>



<h2 class="wp-block-heading"><strong>Signs and symptoms resemble those of other addictions</strong></h2>



<p class="wp-block-paragraph">Increasing numbers of people have been seeking help for what they experience as a problem in their relationship with the internet, gaming (both online and offline) or personal use of electronic devices such as computers, tablets and smart phones. The signs and symptoms causing concern bear a striking similarity to those that characterise addiction to substances such as alcohol, opiates and cocaine and to behaviours like gambling. These include extended hours spent on the internet with increasing evidence of being unable to control the amount of time consumed in cyberspace. This may well be accompanied by excessive preoccupation with the <a href="https://cliniclesalpes.com/blog/internet-addiction/" data-type="link" data-id="https://cliniclesalpes.com/blog/internet-addiction/">internet</a> when not actually on line thereby interfering with a person’s work, study or other leisure activities. Marked changes in temperament, social isolation, increasing tolerance, signs of withdrawal when not using technology or not on the internet and continuing use despite interpersonal conflict over the internet/technology-related behaviour within a family, may all feature.</p>



<h2 class="wp-block-heading"><strong>Availability and ease of access encourage compulsivity</strong></h2>



<p class="wp-block-paragraph">As more people receive help and further studies are completed, researchers and clinicians have begun to consolidate theories as to why internet use and related technologies carry with them the risk of addiction.&nbsp; They point to the endless possibilities for stimulation, arousal and escape combined with the ready availability and ease of access as helping to encourage compulsive behaviour. They highlight the tantalising promise of reward, the rapidity of response (paralleling the “quick fix” of other addictions) and long periods of disconnection from the real world. Add aspects like low cost and anonymity and one can understand the potential for users to become disinhibited and careless of maintaining normal boundaries.</p>



<h2 class="wp-block-heading"><strong>Addicted to the internet, to what it gives access or to the enabling device</strong>?</h2>



<p class="wp-block-paragraph">The debate continues as to whether it is possible to be addicted to the internet and enabling electronic devices rather than to what can be accessed on it, such as gambling, pornography, games, chat rooms or dating sites.&nbsp; However, as in the case of other addictions the context and “route of administration” may be a powerful component of the addictive relationship.&nbsp; The alcohol dependent person is excited by the sight of a bar, the person addicted to heroin may get a buzz from handling a needle and a gambler may be stimulated by encountering a slot machine. A simple way to gauge the degree of unhealthy attachment is to try to give it up for a period and, with support, assess the degree of consequent discomfort.</p>

<h2 class="wp-block-heading"><strong>Increasing numbers of people seeking treatment for Internet Addiction</strong></h2>



<p class="wp-block-paragraph">In any event, as the use of the internet now features heavily in daily life, increasing numbers of people have reported problems with their net-based behaviour. Research across the globe into such matters has accordingly gathered pace and the suffering evidently experienced by some internet users has created a demand for treatment interventions specific to Internet Addiction.</p>



<h2 class="wp-block-heading"><strong>Internet Addiction’s similarities with Substance Addictions</strong></h2>



<p class="wp-block-paragraph">Researchers generally agree that Internet Addiction has striking similarities with addiction to substances. They highlight characteristics of addiction such as:</p>



<ul class="wp-block-list">
<li>A preoccupation with being on-line so that it comes to dominate a person’s thinking around the clock</li>



<li>An increasing amount of time spent on internet-based activity to the exclusion of other interests, with a tendency to stay on line longer than was intended, even to the point of neglecting personal needs. Consequential social disconnection and isolation</li>



<li>An impulse to escape from everyday reality or to seek relief from distress</li>



<li>An increase in tolerance as activity has to increase in order to get the same effect that was originally experienced</li>



<li>Withdrawal symptoms experienced when access is somehow inhibited. The intense discomfort suffered is mostly mood related but may even include physical symptoms</li>



<li>Interpersonal conflict occurs in relation to other people who are close, putting these relationships at risk</li>



<li>Secrecy and dishonesty about the nature and extent of the internet activity</li>



<li>A very real risk of relapse after a period of abstinence or reduced use</li>
</ul>



<h2 class="wp-block-heading"><strong>Electronic devices enable the addiction</strong></h2>



<p class="wp-block-paragraph">Through facilitating instant access to the internet, electronic devices become a critical component of the addictive behaviour and a powerful attachment may be formed to them so that their absence, even for short periods, sufficiently unsettles users to indicate a withdrawal process.</p>

<h2 class="wp-block-heading"><strong>The question of abstinence in treatment for Internet Addiction</strong></h2>



<p class="wp-block-paragraph">Because use of the internet is now so integral to everyday life, the aim of treatment interventions for internet-based addiction is not for the achievement of total abstinence from all on-line activity. Nonetheless, a period of abstinence does help to reveal the severity of the addiction and provides the opportunity to re-set the user’s relationship with the internet.</p>



<p class="wp-block-paragraph">It may well be necessary to maintain abstinence from the area of the internet that is most implicated in addictive behaviour, such as sites for pornography, gambling, gaming or social networking. With the right support, the person can then develop a healthier approach to on-line activity in general.</p>



<p class="wp-block-paragraph">Some people may find that to do so, a residential treatment setting offers a controlled environment where abstinence and/or managed use of internet activity can be productively trialled.</p>
